OPEN TO LEGAL RESIDENTS OF THE FIFTY (50) UNITED STATES AND THE DISTRICT OF COLUMBIA WHO ARE AT LEAST EIGHTEEN (18) YEARS OF AGE OR OLDER AT THE TIME OF ENTRY.

1. HOW IT WORKS: There are two ways to play the Fazoli’s® “Holiday Cash Giveaway” Instant Win Game (the “Game”) which begins on 11/26/12 and ends on 12/12/12 (“Promotion Period”). Participating Fazoli’s restaurants will distribute a Fazoli’s discount card containing various peel and reveal discount offers good at participating Fazoli’s restaurants (“Discount Card”). After peeling all discount offers from your Discount Card, if the Discount Card reveals the word “WINNER” you have won one of 3 denominations of Visa® Gift Cards. If your Discount Card reveals any other message, other than the winning message stated above, or is blank, you are not a winner.

   You may also send a self-addressed, stamped envelope to:

   to be postmarked by 12/1212 and and received not later than 12/17/12. Limit one request per stamped envelope. All requests must be handwritten. No mechanically reproduced requests permitted.

2. PRIZES: We will award a total of 25 prizes each consisting of one Visa Gift Card in one of three dollar amounts: $500 (up to 2 winners), $100 (up to 10 winners), and $50 (up to 13 winners).

3. TO CLAIM A PRIZE: Mail official Discount Card revealing the potential winning message, along with your complete name, street address (no P.O. Boxes) city, state, zip code, phone number and email address, hand-printed on a 3″ x 5″ paper, to:

   Eligible winning Discount Cards must be postmarked by 12/12/12 and received no later than 12/17/12. Verified winners will be notified by mail/phone.

4. VERIFICATION: All claims/redemptions are subject to verification. Decisions of the judges are final. All materials submitted become the sole property of Sponsor, defined below, and will not be returned. Any game card that is illegible, mutilated, altered, duplicated, tampered with, forged, cut, torn, contains printing, typographical, mechanical or other errors, was not obtained legitimately or was distributed where prohibited by law, is null and void. Only game cards obtained via Rule #1 and Rule #3 shall be deemed to have been obtained legitimately. In the event of a printing, production or other error or irregular game card, Fazoli’s System Management, LLC (“”Sponsor”), its parent, affiliates, entities or subsidiaries, its and their members, directors, officers, employees, agents and agencies shall have no liability. If, due to a printing, production or other error, more winning game cards are distributed, or more prizes are claimed/redeemed than are intended to be awarded for any, the intended prizes will be awarded in a random drawing from among all verified prize claims redemptions received. In no event will Sponsor be obligated to award more than the stated number of prizes, or award more than the stated value of the prizes. Game cards may not be transferred or sold. Not responsible for late, lost, damaged, delayed, incomplete, illegible, mutilated, postage due, misdirected mail or mail not received by the deadline set forth in Rule #1 and Rule #3.

5. GENERAL RULES: Open to residents of the 50 United States and District of Columbia (“DC”) aged 18 years or older. Employees of Sponsor, and their parent, affiliates, subsidiaries, advertising and promotion agencies and suppliers involved in this promotion, and the immediate family members (mother, father, sister, brother, husband, wife, daughter, son, step-father, step-mother, step-daughter, step-son, step brother, step-sister, half-brother, half-sister) and/or those living in the same household of each are not eligible. Void where prohibited by law. No substitution, cash redemption or transfer of prize by winners permitted. All federal, state and local taxes are the sole responsibility of winners. All federal, state and local laws and regulations apply. If Prize winner is deemed a minor in the state of his/her residence, prize will be awarded in the name of his/her parent/legal guardian. Return of any prize/prize notification as undeliverable may result in disqualification. Sponsor reserves the right to substitute prize of equal or greater value. Acceptance of prize constitutes permission to the Sponsor to use winner’s name and/or likeness for purposes of advertising and trade without further compensation, unless prohibited by law. By accepting prize, winner (or winner’s parent/legal guardian if winner is deemed a minor in his/her state of residence) agrees to release and hold harmless Sponsor, its parent, affiliates and subsidiaries, and its and their respective members, directors, officers, employees, agents and assigns, from and against any and all claims, damages, losses, expenses and liability, including but not limited to death, arising out of acceptance and/or use of prize. Entrants (or entrant’s parent/legal guardian if entrant is deemed a minor in his/her state of residence) assume all liability for any injury or damage caused, or claimed to be caused, by participation in this Game or the acceptance or use of any prize. By participating in this Game, entrants (or entrant’s parent/legal guardian if any entrant is deemed a minor in the state of his/her residence) agree to be bound by the Official Rules and the decisions of the judges. Sponsor is not responsible for any typographical or other error in the printing, the offering or the announcement of any prize or in the administration of the Game.
